Youth Transitions
Youth with disabilities who are eligible for Individualized Education Programs (IEPs) and who have completed their secondary education are transitioned from a secondary educational setting to a post-secondary educational program.
This service is to help youth with significant disabilities to achieve independence through higher education and employment.
Youth younger than 18 are welcome to join our monthly youth peer support Zoom meetings called Empower Me!
