Assistance is provided in transitioning persons with significant disabilities from an institutional setting such as nursing facilities into a community-based setting.
This maximizes the consumer’s potential to function independently in their own homes.
Disability advocates work together with consumers in setting up a support system including access to community resources.
Transition to Community Living (TCL)
Community Inclusion Specialists at DAC work with referrals from Sandhills who are looking for more support with getting out into the community. They work with consumers to help them find social groups, hobbies, and other resources that help them find a place in their community.
